Purpose This 60-minute intake is designed to gather the information needed to create a personalized, effective care plan tailored to your health goals, medical history, lifestyle, and preferences.
What to Expect (60 minutes)
Welcome & Goals
Brief introduction and confirmation of your primary concerns and goals for care.
Medical & Medication Review
Review of current diagnoses, past medical history, surgeries, allergies.
Comprehensive medication and supplement list, including doses and adherence.
Symptom & Functional Assessment
Detailed discussion of current symptoms, symptom onset and pattern, severity, triggers, and impact on daily life.
Assessment of mobility, sleep, energy, mood, and cognition as relevant.
Lifestyle & Social Determinants
Review of diet, activity/exercise, sleep habits, substance use, work, family and social supports, housing, transportation, and financial or access barriers.
Mental & Emotional Health Screening
Brief screening for anxiety, depression, stressors, coping strategies, and readiness for change.
Goals Prioritization & Patient Preferences
Collaborative discussion to prioritize goals, preferred interventions, cultural or personal preferences, and acceptable levels of intensity/support.
Risk Assessment & Safety Planning (5 minutes)
Identify any safety concerns (fall risk, suicidal ideation, medication risks) and immediate needs; plan next steps if urgent issues arise.
Next Steps & Care Plan Outline (10 minutes)
Summary of findings.
Proposed short-term and long-term goals.
Recommended interventions (medical referrals, medications, therapy, physical therapy, nutrition, sleep, lifestyle changes, community resources).
Timeline, follow-up schedule, and who will coordinate care.
Opportunity for questions and to confirm consent and commitment.

Clinical monitoring and follow-up: We track response to therapy, side effects, lab results, and clinical markers, adjusting the plan in coordination with your prescriber as needed.
Deprescribing when appropriate: We identify medications that may no longer be necessary or that pose increased risk and work with your clinician to taper or discontinue safely.
